Investigation Summary
At approximately 11:40 a.m. On February 21, 2007, Employee #1, a laborer, was in the trench near the street, when the trench walls collapsed engulfing him. Prior to the accident, Employee #1's coworker, a backhoe operator, working for an excavating contractor, opened a trench to allow installation of a sewer pipe. The trench ran approximately 60 ft from one side of a duplex to the street. The trench was approximately 10 ft deep to 11 ft-6in. at the point it collapsed (near the street). The remaining upright portion of the trench revealed the trench walls were almost vertical with the top opening ranging in width from approximately 2 ft to 3.5 ft throughout most of the length of the trench. There was no shoring, bracing, or a trench box in place or in use at the time of the accident. Employee #1 sustained fatal injuries as a result of the accident.
