Investigation Summary
On January 15, 2006, Employee #1 was assisting to thread paper on a take up reel at the end of a paper machine after a paper break occurred. The machine runs at approximately 583 feet per minute and the paper consisted of a heavy paperboard that cereal boxes are made of the weight of the paper makes it difficult to initially thread a new real into the machine. As Employee #1 was loading the paper, a coworker observed that he was getting too close to the machine and yelled for him to step back. However, Employee #1 lifted the paper over his head and threw it down into the nip point, getting one of his hands caught. He was pulled into the machine and was cycled through the machine, ending up caught in the adjacent nip of the spare spool, before the machine could be stopped. Employee #1 suffered multiple injuries including severe head trauma, broken ribs, a punctured lung, a crushed left arm, and amputated left thumb. He later died due to multiple injuries.
