Investigation Summary
On January 16, 2007, Employee #1, an Electrician was found lying on the concrete floor surface of Building Number 15. Employee #1 was assigned the duty of replacing light bulbs. Evidence at the workplace disclosed that he had been replacing burned out light bulbs when he fell to his death. Employee #1 was last seen by a coworker at approximately 12:40 p.m. and he was working alone. The evidence also showed that Employee #1 was on a manlift when he fell. Employee #1 was killed.
