Investigation Summary
On January 29, 2007, Employee #1 finished backfilling guardrail posts. He then began removing the channelizing devices. Starting with the merging taper, he removed the traffic cones over a distance of approximately 2140 ft. With approximately 500 ft remaining, a tractor trailer traveling in the left-hand lane behind the pick-up truck quickly approached the work vehicle and nearly struck the arrow board. Swerving into the median, the semi struck Employee #1 who was killed.
