Employee Is Killed When Struck by Motor Vehicle in Workzone

On October 31, Employee #1, a construction crew's foreman and Employee #2, a tractor trailer driver, were standing in an open lane of traffic when a motorist, who was operating a conversion van in reverse, accelerated and lost control of his vehicle. The van struck Employees #1 and #2. Employee #2 was thrown underneath the trailer and sustained injuries to his neck. Employee #1 was thrown up against the trailer and hit his head. The van then struck the lift truck that was carrying the concrete culvert. The culvert fell from the tines and landed on the roadway next to Employee #1. Employee #1 was taken by ambulance to the hospital, where he was pronounced dead. The employer did not post advance warning signs to alert motorists of the work area. Channelizing devices were not used to redirect the road users' path around the work area. The employees were not wearing high-visibility apparel.
