Investigation Summary

Investigation Nr: 200783793
Event: 02/15/2006
Employee killed in fall through fiberglass roof panel

At approximately 3:10 p.m. on February 15, 2006, Employee #1 was on the roof of a building, removing Christmas lights from the eave. He was walking parallel to the eave, pulling off the light string, when he fell approximately 20 ft through a fiberglass roof panel. He dropped, feet first, through the roof and struck a brick half-wall that was directly beneath the skylight. This caused him to rotate 180 degrees and he landed, head first, on the concrete floor. Employee #1 began bleeding profusely from a head wound and he quickly lost consciousness. He was transported to Parkland Memorial Hospital, where he was pronounced dead.
