Employee's Arm Is Amputated after Being Crushed by Machine

On February 20, 2006, Employee #1, a mold setter, reached into a thermoforming machine that had not been locked out. When a coworker started the machine, the mold closed and crushed Employee #1's arm. The arm was amputated later that day at a hospital.
