Investigation Summary

Investigation Nr: 202077186
Event: 11/10/2005
Employee Is Killed When Asphalt Plant Duct Explodes

Employee #1 was operating a hot mix asphalt plant using a SJPO 1580E Hauck Power Star burner with an Astec 90-005 double-barrel drum mixer, and a RA318P Roto-Aire CMI-Terex baghouse. The asphalt mix was not heating properly and there was an increasing pressure drop across the baghouse. The employee was walking alongside the plant from the mixer toward the baghouse checking for a problem when the exhaust duct between the asphalt mixer and the baghouse exploded. A piece of the duct struck the employee and killed him. Further investigation revealed that the explosion was the result of poor air flow through the hot mix asphalt plant.
