Investigation Summary
At approximately 10:30 a.m. on September 27, 2007, Employee #1, a metal fabricator, completed fabrication of a structural steel boiler pipe support weighing approximately 1,167 pounds and measuring 15 ft by 7 ft. He had previously placed a similar pipe support in temporary storage because the paint line was back logged. The first support was leaned against a press brake. When he placed the next one he removed the hook and sling by climbing on to it. He then stepped away and noticed that it was falling and attempted to move away but was struck, by both pipe supports. Employee #1 was killed.
