Employee's Fingertips Are Amputated by Press

At approximately 12:30 a.m. on August 13, 2007, Employee #1 was operating Williams-White Moline 750 ton press. She reached in to remove a part. As soon as she removed her foot from the foot pedal, it activated again, causing the lifters to go down, amputating the tips of the third and fourth fingers on her right hand. Employee #1 was hospitalized. The slide guarders of the press failed to operate properly in that the foot pedal should be pressed once to activate the lifters up and another signal sent from a safety stand located approximately 5 ft away to activate the lifters to go down. The equipment was set up so both the activations happened with the foot pedal, once to activate the lifters up and secondly to remove the foot from the lifters while it goes back down. Management knew of this but did not inform the employee.
