Employee Is Crushed and Killed While Repairing Machine

At approximately 10:28 p.m. on August 19, 2007, Employee #1 was dislodging a starter bar from the top of the saw clamp and saw assembly on a vertical continuous casting machine. Employee #1 positioned himself on the rear portion of the saw clamp and dislodged the starter bar. The saw clamp and saw assembly had been pushed down to its lowest operating point. Employee #1 did not release the pneumatic pressure on the assembly nor secure the counter weight in a safe position. Once the starter bar was dislodged, the assembly returned up to its normal position. Employee #1 received fatal crushing injuries to the head.
