Investigation Summary
At approximately 7:15 a.m. on November 27, 2006, Employee #1 was working in the filling/packaging department at a cat litter manufacturing plant. He was ascending a fixed ladder to retrieve a defective package from a conveyor. The ladder was damaged and lightly coated with cat litter dust. Employee #1 was killed when he fell from the 12-ft tall ladder and struck his head on the concrete floor.
