Employee's Hand Is Crushed in Machineand Later Amputated

On November 13, 2006, Employee #1 was cleaning a DOA machine Type D1, Serial Number 512-19784. It was standard procedure to clean the machine while it was in operation. Employee #1 reached in to remove the lint and fuzz from the area around the gears and his hand got caught in the gears. Employee #1's right hand was crushed and required amputation. Employee #1 was hospitalized.
