Investigation Summary
At approximately 7:20 p.m. on September 12, 2006, Employee #1 was operating an Edward Segal grommet machine (Model Number G2R83, Serial Number 4132) and had placed the bottom half of a grommet into the unguarded point of operation with her ring finger and thumb when the grommet machine cycled. Employee #1 fractured her ring finger and thumb, and was hospitalized.
