Employee Amputates Fingers While Replacing Machine Belt

At approximately 7:15 p.m. on September 7, 2006, Employee #1 was reinstalling a belt around a pulley on a dust collector. Employee #1 shut off the electric switch controlling the motor but did not lock it out. A coworker saw the dust in the building and noticed the collector was shut off and turned it back on, not knowing it was being worked on. The motor for the system was reenergized. Employee #1 suffered a partial amputation of his index and middle finger. Employee #1 was treated but was not hospitalized.
