Employee's Finger Is Amputated in Machine

At approximately 12:30 a.m. on August 24, 2006, Employee #1, a maintenance worker, was servicing a Novatec vacuum canister located on top of a Unadyn flake dryer at a plastic recycling facility. The machine was not locked out, and the pneumatic slide gate on the canister activated, while his right hand was in the vacuum canister. His right pinky finger was later amputated at the hospital.
