Employee Dies after Being Crushed by Falling Forklift

At approximately 3:40 p.m. on August 17, 2006, Employee #1, a maintenance technician, was working in the maintenance shop of Kenton Iron Products, Inc. He was removing a broken alternator on a Hyster LP forklift, Model Number H50XM, Serial Number H177B28063Y, to replace it with a new one. The NAPA Mobile Professional Service Jack, Model Number 791-6420, Serial Number F0402000229, slipped from underneath the forklift and rolled down the slanted driveway, and the forklift fell on the employee's head. A coworker found Employee #1 lying underneath the 9,309-pound forklift with his head crushed between the bottom of the forklift and the concrete floor. Employee #1 was transported by emergency services to a local hospital and then transferred to St. Vincent Hospital in Toledo, OH, where he died on August 22, 2006. Further investigation revealed that the employer did not have a written standard procedure for this operation and the employer had not provided proper training for performing the task.
