Employee Dies after Being Struck by Unsecured Stack

At approximately 7:30 a.m. on June 2, 2006, Employee #1 was tying the bottoms of storage sacks in a warehouse. An unsecured stack of storage sacks, weighing approximately 500 pounds, fell approximately five feet onto Employee #1. Employee #1 was taken to St. Rita's Hospital, where he was put on life support and died at approximately 10:37 p.m. from a collapsed lung. Company records indicated that there was no work rule or training for the proper storage or stacking of the sacks.
