One Employee Is Killed, One Is Injured by Forklift

On February 24, 2006, Employee #1 and Employee #2, both carpenters, were working at a waste water treatment facility. They were standing on a ramp, approximately 2 ft behind the right rear of a rough terrain forklift, a JLG brand, Model Number 0943A. The operator of the forklift failed to see Employee #1, and Employee #2 as he started to backup the approximately 10-degree inclined ramp. The right rear tire of the forklift backed over Employee #1 and Employee #2's feet were pinned underneath Employee #1. Another worker stopped the forklift operator who then pulled forward. Employee #1 was killed, and Employee #2 fractured his foot.
