Employee Is Crushed and Killed by Hopper Bed

At approximately 7:45 p.m. on October 4, 2005, Employee #1 was disconnecting a wind row elevator from an asphalt lay-down machine. Employee #1 accessed the hitch pins from inside the hopper of the lay-down machine. When Employee #1 removed the second of two pins, the front of the wind row elevator collapsed, pinning him between the hopper bed and the wind row elevator. Employee #1 was not found for approximately 45 minutes and was pronounced dead at the scene.
