Employee Falls from Eleventh Floor Balcony, Later Dies

On December 7, 2006, Employee #1 was working on the 11th floor balcony of a residential building. The building was being renovated, and the employee was applying caulking to the bottom of a sliding glass door. There were vertical lifelines from the roof level that employees were using to attach their lanyards. The balcony was on an end unit and had two unprotected sides. The employee was bent over and stepping backward toward an unprotected edge while applying the caulking. The employee was not tied off and stepped backward off the edge of the balcony. Employee #1 fell 110 ft and died shortly thereafter.
