Investigation Summary

Investigation Nr: 202484309
Event: 07/24/2006
Employee Is Injured When Struck by Roll of Sheet Metal

At approximately 9:50 a.m. on July 24, 2006, Employee #1 was welding under a 27 ft by 8 ft by 0.25-in. sheet metal roll on the "Big Roll," a Kling, 12-ft by 12-in. sheet metal roller. His employer, Perkins Welding/Redding Tanks, Incorporated, manufactures steel tanks. During welding of the seam, the roll was supported by a CM Lodestar Electric Chain Hoist rated for 3-tons. Employee #1 had hooked the roll to the hoist and set vice grips to hold the sheet metal prior to welding the seam. He was injured when the rolled steel shifted on the hooks, the vice grips that were holding the seam popped off, and the steel un-rolled and struck him. He was treated for injuries to his back, chest and shoulders.
