Investigation Summary

Investigation Nr: 201179504
Event: 04/04/2006
Employee Fractures Ankles When Struck by Moving Steel I-Beam

On April 4, 2006, Employee #1 was stacking steel I-beams with an overhead crane. The beams were on a raised platform closes in proximity to the facility's drill and burn lines. Underneath the platform, there was a conveyor system that allowed operators to lift and move steel members around the large platform. As Employee #1 was preparing to stack the beams, the drill line operator activated the conveying system. Once activated, the beams were elevated and moved towards Employee #1. Employee #1 was not within the cab of overhead crane, but standing on the platform near the conveyor system. He was struck by a passing steel beam. The beam pinned his feet between the beam and platform behind him, fracturing both ankles. Emergency medical personnel were summoned. They arrived and transported him to a medical center. At the hospital, he underwent surgical procedures to repair the multiple fractures. After undergoing the surgical procedures, he was hospitalized for postoperative treatment and was released nine days later.
