Investigation Summary
On December 6, 2005, Employee #1 was cleaning the covers of the screw conveyor troughs in the head-house at the top of the rice drying silos, at the De Pue Warehouse Company. He ducked under an overhead framing member at the same time that he stepped over a channel iron that was lying across the conveyor cover. He was injured when he stepped on the cover and it flipped, causing both his foot and the cover to fall into the moving conveyor. The cover and Employee #1's leg became entangled in the conveyor, causing the conveyor to stop. He was hospitalized at Enloe Medical Center in Chico, for nerve damage and severe lacerations to several areas of his right leg and foot.
