Employee Is Injured During Elevator Maintenance Job

On January 30, 2006, Employee #1, working for ACC-GWG LLC, was part of a crew servicing a rice mill elevator system. The elevator belt was removed to reveal the top pulley and belt, and the system was locked out. The safety guard associated with the drive components of the top pulley was removed. Employee #1 grabbed the belt of the drive system and started applying tension by pulling it. As he continued pulling, the belt tension was suddenly released and one of his left hand fingers became entangled in the belt and pulley, causing a distal amputation. The employee drove himself to Colusa Regional Medical Center, in Colusa, CA. He was released from the hospital several hours after being admitted.
