Employee's Foot Is Crushed by Scissor Lift Table

On August 17, 2005, Employee #1, a stacker, was working at a scissor lilt table used to place OSB on, after it has been ripped via an automated rip saw. Employee #1 had stacked approximately three-fourths of a unit of OSB. After stacking another ripped sheet of 0.5 inch OSB, the hydraulic lift table came down. When the lift table came down, his foot was caught where the shearing and pinching action existed between the lift table and the standing platform, causing amputation of his large toe and partial amputation of two others. He was hospitalized for treatment.
