Employee Fractures Foot When He Overturns a Forklift

At approximately 11:30 a.m. on October 19, 2005, Employee #1 was backing his forklift into position to pick up a large bale of material. The forks were elevated to the top of the mast making the vehicle top heavy. The vehicle was being operated at a speed too fast for even good conditions, due to Employee #1's inexperience. When he turned the forklift it tipped over. He was not wearing his seatbelt and was partially thrown from the protective cab, roll over protection device. His left foot was crushed between the vehicle and the concrete floor. He was hospitalized with a fractured foot.
