Investigation Summary
On November 1, 2005, Employee #1, a maintenance man, was adjusting the limit-switch wire of a Littell automatic centering reel, Model Number 60, Serial Number S9194-79, while it was in the ON position. As Employee #1 was located at the end of the machine, reaching between the keepers and its base, the machine cycled. He became pinned with his head crushed between the base and the keepers of the centering reel. Employee #1 was killed.
