Investigation Summary

Investigation Nr: 200354983
Event: 11/07/2005
Employee Killed When Scissor Lift Tips Over

Employee #1 and a coworker were working from a JLG scissors lift, 25.5 ft above the concrete floor removing steel racks from a warehouse that was being renovated. The tops of the frames were attached to the upper guardrails of the extended personnel platform with a cotton rope. The employees cut the bolts holding the 30-ft by 10-ft frames. The rope was let out slowly to lower the frame. The frame being lowered on the right side of the scaffold struck another steel frame which changed from the lift's center of gravity. The lift tipped to the left and then fell to the concrete floor. Employee #1 was killed when his chest struck the lift's upper guardrail causing internal injuries. The coworker was hospitalized for treatment of lacerations to his liver and right kidney, broken T4-T6 vertebra, and broken left ankle.
