Investigation Summary
At approximately 6:00 a.m. on February 23, 2009, an employee was instructed by his immediate supervisor to grind and polish acrylic material using the Porter Machine Company B-Grinder located in Building Number 1, Department 50, Grind and Polish Department. The employee started work at approximately 6:15 a.m. and during the course of his work, needed to change the grinding pad. Around 9:00 a.m., the employee changed the grinding pad by stopping the machine, loosening the steel collar and changing out the grinding pad. After changing the pad, the employee ran the B-Grinder again and noticed that the collar was loose. The employee decided to tighten the collar using his left hand while his right hand was resting to the right of the collar. The machine arm came towards the employee's right hand, and crushed his hand between the arm of the B-Grinder and the area to the right of the collar. The employee was taken to a nearby hospital by ambulance. He was hospitalized and treated for an avulsion injury of the right index, right middle, right ring, and right fifth fingers neurovascular bundles.
