Investigation Summary
On November 13, 2008, Employee #1, a mill operator, was changing the small drive belt on a Cincinnati OD Milling Machine, Serial Number 3U4D1Z-7. He was manually guiding the belt onto the drive pulley, when his middle finger was pinched and crushed between the belt and the pulley. His finger tip was amputated and he was hospitalized with his injury. Further investigation revealed that the mill breaker box was turned off, locked, and tagged.
