Investigation Summary
On October 30, 2008, Employee #1 was using newspaper to clean the rotating transport bands inside a machine. As he cleaned the bands, his hand was pulled into the machine's rotating chains and sprocket. The moving parts amputated his right hand's ring finger from the proximal to middle phalanx. He was transported to a medical center. At the hospital, he received medical attention and was released.
