Investigation Summary
On December 8, 2008, Employee #1, an apprentice operator for an asphalt manufacturer, was assigned by his supervisor to tend the crusher plant. The crusher plant consists of a series of conveyor belts that carry material to the crusher and then to a storage location or through to processing. While the crusher plant was in operation, Employee #1 began cleaning up gravel and powder that had fallen off of the underscreen conveyor. He then mounted a stepstool to clean debris from the conveyor frame, using a shovel. As Employee #1 made one pass along the frame in an attempt to remove material close to the pulley, which was unguarded, the shovel he was holding became entangled in the belt and his right arm was pulled between the belt and pulley. Employee #1 was taken to the nearest medical center, where he was hospitalized and underwent surgery on his right arm. However, the tip of his right index finger was amputated.
