Employee's Fingers Are Amputated in Mill Machine

At approximately 5:00 a.m. on December 12, 2008, Employee #1 was instructed by his supervisor to assist a coworker as a helper on Extruder Line Number 1. Employee # 1 was instructed to take finished rubber products off line Number 1 and place them into a plastic bin. Hours later, Employee # 1 was instructed to use Mill Machine Number 1 to grind rubber for the next rubber product. Employee # 1 was placing a folded sheet of rubber into Mill Machine Number 1 and his cotton-milling glove was caught between the in running rollers and the folded rubber sheet. Employee # 1 pulled his hand back and pulled down the safety bar. He was hospitalized with four amputated fingers.
