Employee fractures his knee while tripping over a pallet jac

On November 4, 2008, Employee #1, a production line lead, was supervising a group of workers assembling gift baskets. Employee #2, his direct supervisor, was in the workplace that day but was not present at the time of the accident. Employee #1 was returning to the production area with some supplies for the baskets under assembly when he encountered employee #3 slowly walking a pallet jack across the aisle. Employee #1 stepped over the fork nearest him, and then tried to step over the second fork of the pallet jack. However, his right foot caught on the fork, and he tripped and fell on his left knee, twisting his left leg as he fell. He was taken for emergency care at a local occupational injury clinic. The following day he underwent surgery for a fractured left knee patella.
