Employee gets fingers crushed and amputated

On November 5, 2008, Employee #1, a machine operator, was assigned by Employee #2 to run small parts 1.5 inches by 4 inches on a Part Revolution Punch Press manufactured by Minster Co, Model No 6. Employee #1 had run several parts by placing the part into the point of operation then using a 2-hand control to activate the machine. Employee #1 then took the part out by hand. As Employee #1 took the part that was worked out of the point of operation, the punch press cycled on its own without depressing the 2-hand controls. The punch caught Employee #1 right hand crushing her fingers resulting in amputations and crushing injuries. Employee #1 was hospitalized for 2 days to receive surgery to repair the injuries to her hand.
