Employee Fractures Foot in Pallet Truck Accident

On September 26, 2008, Employee #1 had been working for two and one-half weeks as a temporary employee, at a business that manufactures assembled and unassembled cabinets for home use and ships them to retail stores. Employee #1 was performing his regular duties of pulling orders in the warehouse and staging them at the loading dock, for loading onto trailers for highway shipment. Employee #1's supervisor gave him the orders. To take the orders to the dock, Employee #1 was using a Toyota Electric Ride-on Pallet Jack Truck (Model Number 7HBE30, Serial Number 7H8830-31953). On one trip to deliver a pallet of goods for staging at the dock, Employee #1, who was riding on the pallet jack, tried to turn the pallet jack, to avoid pallets staged near the warehouse door. He swung wide, however, and approached too closely to the pallets. As he turned, his right foot slipped off the pallet jack and was crushed between the pallet jack and the pallets. Employee #1 was taken to a local medical center, where he was hospitalized and underwent surgery on his fractured foot.
