Investigation Summary
On June 21, 2008, an employee was operating an end fiber machine, Parker Series 2MA, Model Number 02.50 C2MAU34A 6.000, Serial Number. HH299536A. The machine used a pneumatically powered piston to slide plastic parts, spider ends, onto the metal shaft of an armature. The employee was removing a completed armature that had become stuck in the bottom die of the machine. With his right hand, he grabbed the metal rod of the armature using his thumb and index fingers. The tips of his right middle and ring fingers rested on the top edge of the bottom die. The employee's left hand touched the left side button of the two-hand actuator control, MEAD, Model CSV-102, Anti-Tie Down, causing the piston to come down and crush his finger. The employee suffered a right middle finger distal tip amputation. He was transported to the hospital and treated for his injuries and released.
