Employee Fractures Wrist and Forearm in Fall from Ladder

At approximately 5:00 a.m. on July 14, 2008, Employee #1 worked at a theme park, which includes a resort hotel and its associated restaurants. Employee #1 and coworkers were instructed to take down windows, doors, decor, and moldings. Employee # 1 grabbed a brand new 20 ft, type 1A Green Bull Extension Ladder, Model Number 610120, and set it up against a wall. He climbed the ladder and began removing screws that held up the window shade located approximately 13 ft from the ground. He lost his balance; fell onto the concrete, fracturing his wrist and forearm. Employee #1 was hospitalized.
