Investigation Summary
On February 12, 2008, Employee #1 and a coworker were working in the warehouse, placing storage items into wooden crates located on the floor. Employee #1 placed a 40-ft extension ladder against the south wall of the warehouse to access a sofa that was being stored at a height of 17 feet. The ladder was not fully extended, and the length at the time of use was 14 feet. Employee #1 ascended the ladder while the coworker held the bottom. He ascended and descended facing the ladder and holding onto it. While descending, Employee #1 stepped off of the right side of the ladder with his right foot at a height of 9 feet. He fell, but struck a sofa before landing on the floor. The sofa was located on the floor between the feet of the ladder and the wall. Employee #1 suffered a scalp laceration, concussion and fractured ribs/vertebrae. He was transported to the hospital for treatment.
