Investigation Summary

On June 7, 2008, Employee #1, the immediate supervisor and full-time employee of a drywall contractor, was loading drywall and mud boxes onto the second level of an apartment complex, with Employee #2. The scaffold rails had been removed to provide access into the building. As Employee #1 started to replace the scaffold railings, he lost his balance. Rather than taking a fall, he jumped off the scaffold and landed on his feet, on asphalt, 12 feet below. Employee #1 apparently fractured his foot and was hospitalized. Employee #2 was in the building and did not see the accident. No one else witnessed the accident. The accident was reported to DOSH on June 7, 2008.
