Employee Is Killed When Run over by Forklift

On April 28, 2008, Employee #1 was a forklift operator for a masonry products manufacturer. Employee #1 transported a pallet loaded with concrete blocks from the manufacturing site to an adjacent storage yard. Coworkers heard the forklift back-up alarm and noticed that the Johnson/Hyster Model No. 155 forklift was creeping backwards without the operator. Employee #1 was killed when he was run over by the forklift and his head was crushed.
