Investigation Summary
On March 30, 2008, Employee #1 was cleaning a FEDCO piston depositor, Model Number PF-40-TL, Serial Number 200085, that was part of a commercial bakery. The depositor was located in the oven department of the production area and was used to dispense portions of batter for baking. Employee #1 disengaged the pneumatic power, lowered the depositor batter hopper, lifted the hopper lid, and then disengaged the power to the metering chamber at the base of the hopper. Employee #1 then reached into the hopper near the batter metering chamber with his right hand to begin cleaning. However, Employee #1 unintentionally activated the machines by-pass (hopper-lid interlock) with his left hand that was located near the top edge of the hopper. Power was restored to the metering chamber that began rotating. Employee #1's right middle- and index-fingers were amputated. Employee #1 was taken to a nearby medical facility, where he was hospitalized and treated for his injuries.
