Investigation Summary
On February 29, 2008, Employee #1 was refurbishing an antique chair. He was using a table saw without a guard (Powermatic, Model 66, s/n 8866347, Baldor Industrial Motor, Frame 1840, 5 HP, 3450 RPM, Magnate saw blade, Ref. No. 11-01006001, Max. 7000, 10" circular blade), and was making a cut on a piece of wood that was about 3" by 3" by 1". Employee #1 was holding the piece of wood in his two hands, and was not using the push stick(s) that were available at this workplace. Employee #1 injured his middle and ring finger on his left hand when he accidentally contacted the unguarded saw blade. He was hospitalized for 15 days, and had surgery to amputate the left hand's middle and ring finger (both between the first and second knuckle) as a result of this industrial injury.
