Employee Amputates Finger in Machine Conveyor Roller

On August 5, 2007, Employee #1 was assigned to clean circuit boards by placing them in a scrubber manufactured by Ishii-Hyoki. The employer is engaged in manufacturing printed circuit boards. The scrubber has a slurry tank of aluminum oxide, measuring approximately 52-inches in diameter, located about eight inches under a waist-high box-like conveyor. Hoses from the tank feed aluminum oxide slurry (next to the conveyor box) are use to jet spray clean the circuit boards as they are carried by the conveyor. A horizontally-oriented double strand 0.375-inch v-belt and pulley was located on top of the body of the tank. This powered belt and pulley mechanism rotates an arm within the tank to keep the slurry suspended. Employee #1 left his work station to bring back aluminum oxide for the slurry mix. Upon returning to the machine, he tripped and fell. As Employee #1 was falling, he put his hands out to catch himself. His right hand went behind a partial guard and entered the moving belt and pulley drive mechanism. Employee #1 pulled his hand out and saw that the tip of his right pinky finger was amputated at the distal joint. Employee #1 was released from the hospital after treatment for 28 hours due to an amputation and an open fracture of his right fifth finger tip.
