Employee's Finger Is Amputated in Milling Machine

On May 8, 2007, Employee #1 was running a programmable milling machine manufactured by Trak, Model K3, and Serial Number 022BS11466. The milling machine is capable of being run manually but was in the programmable mode. Employee #1 was milling a metal piece and needed to change the cutting tool. This happens approximately 4 to 5 times a day depending on the piece being worked on. His left hand was holding onto the chuck and cutting tool, when he reached with his right hand to the computer control panel, located immediately to the right of the milling machine. He entered a wrong program that caused the machine to go into rapid mode. The top part of the milling machine moved laterally, and his finger was crushed against the fixture holding the stock being milled and the tool. Employee #1's finger was amputated.
