Investigation Summary
On June 14, 2007, an employee and two coworkers were painting the exterior of a two-story multiplex home. The employee was standing on a Werner 24-foot fiberglass extension ladder, which had been extended to a length of approximately 24-ft, spray painting the second-story eaves of the building. The ladder was not secured at either the top or bottom, and the base had been placed on a 30-in. wide sidewalk alongside the house, causing its pitch to be very steep. The employer, his direct supervisor, had been at the site earlier in the day, before work had started, but was not at the site at the time of the accident. A coworker, who had been gathering materials inside the garage area, walked to the side of the house where the employee had just started working when he heard a crash and found the ladder on the ground and the employee lying on his back on the concrete sidewalk, injured. Emergency response personnel took the employee to the trauma unit of the local medical center, where he was admitted for treatment for multiple fractures on the right side of his face.
