Employee Is Injured in Fall from Forklift

At approximately 11:45 a.m. on February 12, 2007, an employee was directed by his supervisor to cover a pile of lumber with a tarp for protection against the rain. A coworker was directed to raise the employee on top of the stacked lumber by using a forklift truck, Toyota, Johnson Lift/Hyster, Model Number 52-6FGCU45, Serial Number 61274. Both the employee and the coworker were located in the southeast corner exterior of the building. Upon completion, The employee was being lowered to the asphalt ground as one of the forks from the forklift became dislodged and the employee fell approximately 8 feet, hit his side on the remaining fork, and landed on the asphalt ground. The employee sustained serious internal and head injuries, resulting in hospitalization for three weeks.
