Two Employees Are Exposed to Electrical Arc Flash

On December 18, 2006, Employee #1 and Employee #2 worked at a grocery store in the maintenance department. The electrical power was out in some parts of the warehouse building. Employee #1 and Employee #2 were determining the cause of the power outage. They were checking the voltage levels on the different parts of a 480 V panel with a voltmeter. Employee #1 was holding the voltmeter in his hand while Employee #2 was contacting different parts of the circuits with the voltmeter leads. Employee #2 shorted an energized part of the circuit to the metal cabinet housing this circuit. It created a flash and explosion, causing Employee #1 to drop the meter and both were thrown back. Employee #1 and Employee #2 suffered electrical shock with no visible burn marks. In the hospital, doctors noticed an entry point for electricity on Employee #2's right hand but no exit point. There was no entry or exit point on Employee #1. They were both hospitalized for two days.
