Investigation Summary
On February 8, 2007, Employee #1 was working as a full-time payroll employee with no other contracts for a general contractor that had contracted to renovate a residential home. He was hanging a tarp from a second-story roof to cover two windows on the second story, when he slipped and fell onto the first-story roof and then fell again onto the ground below. Employee #1 sustained abdominal pain, spine pain, and a fracture in his left leg. He was hospitalized.
